Agreement
Confidentiality is of utmost importance at the Uptown
Shelter. All information about current or former residents
or any information seen in files or overheard must be kept
confidential. No information can be divulged to
unauthorized personnel inside or outside the Shelter.
Violation of this policy could result in either civil action
for the collection of monetary damages, and or suspension or
dismissal.
All
volunteers must sign our Confidentiality Statement during
the volunteer application process and prior to volunteering
at the Shelter. Our Confidentiality Statement is
listed below.
CONFIDENTIALITY STATEMENT
In connection with my volunteer activities with
the Uptown Shelter, I agree to hold all information about
Shelter clients or former clients confidential and will not
divulge any information to unauthorized personnel inside or
outside the Uptown Shelter. I further agree that I will not
remove any information and/or records regarding Uptown
Shelter clients. I understand that if I divulge
confidential information to unauthorized persons, I am
subject to either civil action for the collection of
monetary damages, and/or suspension or dismissal. In order
to safeguard Uptown Shelter client information, I understand
that I will not discuss or release any information regarding
Shelter clients. I have read, understand, and agree to
comply with the above statements and Volunteer Agreement.
